Hypokalemia

Manifestations Treatment Disposition
  • Decrease GI motility

  • Neuromuscular (<2.5 mEq/l):

    • Lethargy,

    • Weakness,

    • Decreased DTRs

    • Rhabdomyolysis.

  • Cardiac/EKG        

    • Dysrhythmias (various)

    • Decreased. T,

    • ST depression,

    • Prolongation of QRS,

    • Increased U

  • 30% also have hypomagnesemia

  • Estimate Deficit:

    • Approximately 100 mEq deficit for each 0.5 mEq/L decrease in serum K

  • Oral replacement preferred (>2.5 mEq/l, no EKG changes and asymptomatic)

  • IV replacment

    • Rate not > 40 mEq/hr

    • Cardiac monitor

    • Concentration not > 10 mEq per 100cc

(Caution in presence of renal insufficiency / failure)

     

  • Dependent upon:

    • Magnitude

    • Cause

    • Underlying illnesses

    • Response to ED therapy
